When RTF is generated the first time, the design is far from correct compared with HTML.
When clicking 'Generate from HTML' button, the design is correct.

Solution
Auto generating RTF the first time must be eliminated - or same function as 'Generate from HTML' must be used.

WORKAROUND
Delete RTF in code view and click 'Generate from HTML' to get correct design in RTF.

Generate from HTML button will add borders to any signature using table design.

As we are not developing an RTF editor, we have removed the default auto generation features and added some text that explains how to use the RTF editor.
